Find Disciplinary Actions
Through our searchable database consumers can verify the professional standings of individuals or facilities licensed by the Department of Health to provide service or care. Consumers can learn about the type of professional violations committed by licensees, when these violations took place, and the disciplinary actions taken in response. Actions that are under investigation are not included in this database. Information about investigations of and compliance orders issued to hospitals and nursing homes are not accessible through this search.
