What You Should Do to Apply for a License

Individual applications have specific information about licensing requirements and the application process. We license professionals, facilities, organizations and equipment in a variety of areas including: healthcare, cosmetology & beauty, environmental health, restaurants & food , drinking water and radiation control.

Take an exam, if required

You may need to pass an exam in order to apply for your license. Check your rules and regulations and license application for more information. Exams for certain licenses are given at the Department of Health.

Complete your application

We provide most applications as fill-in Adode Acrobat forms, which allow you to type in most of the information from your computer prior to printing and signing the form. If you have trouble with these forms, please contact us.

Submit your application

Print and and mail your application to the address noted on the application. The Department requires original signatures for initial license applications and cannot accept emailed or faxed forms.
